Change of Address

Please note licensees have two different types of addresses:

  • Address of record is the address the Board sends most correspondence to including renewal reminders.
  • Place of practice addresses are locations where acupuncture is practiced. A licensee may have one or more place of practice locations. See below for instructions for the address type you are trying to update.

Address of Record

As per and California Code of Regulations Section 1399.406, you must notify the Board of your new address within 30 days.

To complete your address change using a paper application, please download, print out, and fill out the address change form. Once completed, return the form to the Board.

Place of Practice (Wall License)

As per California Business Professions Code Section 4961, you must obtain a wall license for each place of practice.

Wall licenses are non-transferable. You will need to cancel wall licenses for addresses at which you no longer practice acupuncture. You must also register any new practice addresses for a wall license. Each new wall license is $50.

If you want to submit a paper application, you need to complete the wall license location form. Once completed, return the form to the Board.
